Monthly Checkout Procedures

During the year you will need to complete the monthly checkout procedure as follows:

  • Form 8 - OEC Change Request Form should be completed and sent to the OEC office if changes have occurred since the previous month.
  • Form 12 - Alternative Assessment Roster should be updated if new students are added since the previous month.
  • Special Education Placement and Services Validation - Please submit each month to the OEC office. (Please highlight any changes.)
  • Indicator 13 Checklist - (Completed for students age 14+) Submit these to the OEC office.

The Office of Exceptional Children will forward to each Principal and Assistant Principal an Anniversary IEP Calendar for reference, and a VAAP and VGLA Participations Roster to be validated monthly. If you have any questions, please call Mrs. Bohannon at 434.799.6400x260. Thank you.
