Below is the Google Form to request a Green Card, you may also use the following link: https://forms.gle/zm31chk6bpy31iyW8. You are strongly encouraged to wait until you need "Behind the Wheel" to pick up the Green Card as there is a replacement fee for duplicate copies if lost.
Before Requesting a Green Card, please note the following:
IF YOU ARE 18 YEARS OF AGE AND HAVE HELD A LEARNER'S PERMIT FOR AT LEAST 60 DAYS, YOU DO NOT NEED A GREEN CARD TO TAKE THE ROAD SKILLS TEST AT THE DMV.
Our records only date back to August 2016. If you took driver's ed before, then we will not be able to process a Green Card for you.
Green Cards are only necessary for:
-A person under the age of 18 to take Behind the Wheel lessons with a private instructor.
-A person who has failed the learner's permit exam at the DMV three consecutive times and wishes to retest.
Please fill out this form to request a green card. Once your green card is ready for pickup, you will receive an email indicating when and where you can pick it up; or what you need to do in order to receive it.
GREEN CARDS ARE ONLY WRITTEN WHEN SCHOOL IS IN SESSION FROM AUGUST TO MAY.
