Danville Public Schools Maintenance Request Policy

All requests for Maintenance Services:

1) Complete a Requisition Form outlining the problem, exact location, and specific need. This form should include as much information as possible so the request can be evaluated.

2) Return the completed form to the principal/site administrator.

3) Principal/site administrator will review request and discuss the request with the head custodian to see if the request can be completed in house by the custodial staff. The administrator will then review the request in regards to capital needs, the practical nature of the request, and district policy.

4) Administrator will sign all approved forms and forward to maintenance by inter office mail or fax. Administrator should keep a copy of the request.

5) Maintenance personnel will review the request and establish a computerized work order or reject the request and return it to the administrator with the reason it was rejected.

6) Maintenance requests will be assigned according to health and safety needs, available funding, and district capital plans.

7) All requests for after hours heat/AC must be on a “After Hours HVAC Request For Services” form, signed by the site administrator and faxed to maintenance.
